About Us

Unite Florida is a coordinated care network of health and social care providers. Partners in the network are connected through a shared technology platform, Unite Us, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities. Unite Florida is sponsored by CVS Health, Aetna Better Health of Florida, the Health Care District of Palm Beach County, and the Charles and Margery Barancik Foundation implemented with Sarasota Memorial Health Care System.

Joining the network is free of cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers.

Community Roots

Unite Florida currently serves Pinellas, Hillsborough, Polk, Manatee, Sarasota and Palm Beach Counties. We will have a rapid expansion into Northeast Florida, South Florida and Central Florida this year.

UnitePBC

Sponsored by the Health Care District of Palm Beach County, a unique and diverse public safety-net healthcare system, UnitePBC is a social care network serving community members in Palm Beach County. Since February 2020, this local network expands the geography and impact of Unite Florida.

First 1,000 Days Sarasota County

First 1,000 Days Sarasota County is an initiative comprised 68 community organizations all with a common goal to improve the coordination of services and increase access to care for pregnant women and families with young children. By pulling together our partner organizations’ resources and working through system-wide barriers, this initiative provides better access to the prenatal, newborn and early childhood care that gives babies and families a crucial opportunity to thrive. The initiative was spearheaded by the Charles and Margery Barancik Foundation, implemented with Sarasota Memorial Health Care System as the backbone organization, and supported through generous donations of local foundations and individual donors who value the life-changing importance of investing in prenatal care and early childhood development.

The Health Care District of Palm Beach County is proud to help link residents in need to vital social services, like nutritious food, medical care and a safe place to live. Unite PBC connects the county’s many outstanding community organizations that deliver services that address the social determinants of health. One of the best features of this platform is that by tracking and measuring outcomes, our partners receive confirmation that clients are getting the important services they need.

Having a baby is a life-changing event. Even for the best-supported families, it can “take a village.” Unite Us is one of the key solutions for the First 1,000 Days initiative, to ensure families have the support they need to help their children thrive. It is not only an outcomes-focused, closed loop electronic referral platform, but provides an accurate view of resources and a real-time data dashboard to view gaps in services and where to focus our initiative efforts. The launch of Unite Us in Sarasota County was extraordinarily successful, which speaks to the power of partnerships in our community.
